99201 Office Visit, New Patient, low complexity: The provider sees a new patient for an office visit or other outpatient visit. The visit involves a problem focused history; a problem focused examination; and straightforward medical decision making.
99202 Office Visit, New Patient – low complexity, 15-29 minutes: The provider sees a new patient for an office visit or other outpatient visit. The visit involves an expanded problem focused history; an expanded problem focused examination; and straightforward medical decision making. The provider spends 15–29 minutes of total time on the encounter on a single date.
99203 Office Visit, New Patient – low complexity, 30-44 minutes: The provider sees a new patient for an office visit or other outpatient visit. The visit involves a detailed history; a detailed examination; and medical decision making of low complexity. The provider spends 30–44 minutes of total time on the encounter on a single date.
99204 Office Visit, New Patient – moderate complexity, 45-59 minutes: The provider sees a new patient for an office visit or other outpatient visit. The visit involves a comprehensive history; a comprehensive examination; and medical decision making of moderate complexity. The provider spends 45–59 minutes of total time on the encounter on a single date.
99205 Office Visit, New Patient – high complexity, 60-74 minutes: The provider sees a new patient for an office visit or other outpatient visit. The visit involves a comprehensive history; a comprehensive examination; and medical decision making of high complexity. The provider spends 60–74 minutes of total time on the encounter on a single date.
99211 Office Visit, Established Patient – low complexity, 5 minutes: An established patient has presenting problem(s) that are minimal. Typically, 5 minutes are spent performing or supervising these services.
99212 Office Visit, Established Patient – low complexity, 10-19 minutes: The provider sees an established patient for an office visit or other outpatient visit. The visit involves at least two of these three key components: a problem focused history; a problem focused examination; straightforward medical decision making.
